To prepare a Canadian copyright application, the following information is required:

  1. the full name and post office address of the copyright owner (including the province, state or country of incorporation, if the applicant is a corporation), and complete information on how ownership was acquired;
  2. the full name, post office address, and citizenship of each author (this may sometimes be waived if the names of the authors are unknown but the authors are employed by the copyright owner);
  3. the title of the work;
  4. a description of the type of work, namely, artistic, literary, musical, dramatic, or any combination, as the case may be;
  5. the date and place of first publication of the work (i.e., the date and place at which copies of the work were first distributed to the public), or a statement that the work has not been published.

In Canada, copies of the copyright work are not required to be filed and will not be accepted by the Copyright Office. However, it is usually desirable for the author to retain a copy of the work in a safe place with the certificate of copyright registration so that if a question ever arises as to the content of the work in which the copyright subsists, the filed documents will remove any doubt. Because of the possibility of death or disability or succession of corporate office, it may also be desirable to execute an affidavit establishing that the certificate relates to an identified work exhibited to the affidavit.
